Q: How do I place an order?

Q: When may I contact Customer Service?
A: Our Customer Service Department is staffed from 6:00 a.m. to 5:00 p.m. Pacific Standard Time. However, messages may be sent via fax, e-mail or voicemail 24/7. Should you decide to use any one of these methods outside our normal business hours, we will contact your during our next regularly scheduled business day.

Q: How do I check the status of my order or make changes or amendments?
A: Send or call us with your Purchase Order Number, your name, name of hospital, return contact information, item(s) and quantity, what you need done, and/or any helpful details.

Q: How do I return something?
A: Any return to Mizuho OSI must be accompanied with a Return Authorization Number (RGA#). All returns are authorized by Mizuho OSI Customer Service. We will issue a RGA Number (Return Authorization Number) with the address for the returning material. For an Exchange Of Material or a Return For Credit, we will need to know the Purchase Order Number, the date of purchase, a description of what your require, Part Number, Quantity, whether or not we should reship using the same or new Purchase Order Number, your name, name of hospital, contact information, shipping address, condition of material (unused).

A: Any concern of yours is a concern of Mizuho OSI Customer Service. If we donÍt have an immediate answer, we will help you find one by connecting you to the right individual/department. We continually strive to improve our service and our products. The members of our Customer Service Department have over 34 years experience servicing Mizuho OSI customers.

Q: Where do I get an instruction manual for an Mizuho OSI product?
A: Contact our Customer Service Department with the model number and serial number of the product. Be specific by including all suffix and prefix numbers and letters.

Q: How do I find my Sales Representative?
A: Mizuho OSI Customer Service will provide you with that information.

Q: My shipment has not arrived, how do I track it?
A: Contact Customer Service. We will track your shipment for you.

The Mizuho OSI Guarantee
Mizuho OSI GUARANTEES all of its products for one full year from date of invoice for defects in material and workmanship with the exception of products which are obsolete, misused, altered, damaged, specialty, or custom made. Defective merchandise will be credited or replaced.
